After 2 unsuccessful physical therapy attempts, this pitcher was referred to @MVP_SportsRehab. He previously could not pitch due to elbow pain (UCL partial tear). During the rehab process at MVP he hit a velocity PR and recently made his high school varsity debut.
Preventing ⚾️ injuries requires a case by case approach. If you’re trying to solve the injury problem as a whole across all ages and skill levels you will be on an endless search for answers.

The best ⚾️ rehabs are… Guided by flexible plans, not strict protocols… Progressed by stress, not exercises… Include throwing during the rehab, not after therapy has “ended”.

The ⚾️ players I work with consistently over months/years begin to ask really good questions about their health. In my opinion, it’s this kind of interaction and relationship that can TRULY impact baseball careers and the rise in injuries. Not an exercise here or there.
⚾️ Return to Throwing Programs… Be flexible and adjust constantly. Outside of intensity, volume, and distance here are some variables that play a major role👇 🔘Nutrition 🔘Sleep 🔘Continued strength training 🔘School 🔘Other sports 🔘Extracurriculars 🔘Relationships
Working the kinetic chain is great, but don’t forget to load injured tissue. Build capacity to throw. You can do all the mechanical drill work you want. No drill work is going to overcome a 30% rotator cuff strength deficit.
Sports injuries are complex. Be careful of the providers (especially on social media) who market the “quick fix”. There isn’t one exercise that can solve your problem. Fixing the actual problem requires a well rounded, all encompassing rehab plan.
Low back pain ⚾️ player - “My back felt better after doing back extensions.” Too often healthcare providers prescribe plans that AVOID the injured tissue/areas. Injured tissue is weak. We should LOAD that tissue, and build its capacity to handle the demands of the sport.
Tommy John… The long recovery time really boils down to one thing… The graft. A tendon is supposed to replace and become a ligament 🤯. Those are two entirely different types of tissues. That maturation process can take up to 2 years.
I've tried all advanced methods you can think of: - Bands & Chains - Specialty Bars - Triphasic, Machines - VBT - Clusters - Squat 5x/week You name it. The best method for 99% of the athletes you'll work with is just progressive overload each week and consistency. Stacking…

This isn't a knock on the dad, it just shows how low the knowledge floor is and we can go a LONG ways with some basic education.
The greatest sin “evidence-based” practitioners commit: Ignoring the data happening in real time, right under their nose, in favor of data someone else collected. Phrased another way: They make decisions on the how things “should” be, rather than how they are.
If we're being honest, the likelihood of reducing youth injuries, especially throwing-related, is exceedingly low. This is because the things that would actually help reduce injuries - fewer games, enforcement of pitch count guidelines, having more kids in a team pitch, etc.....

Exercise Order matters Follow this : - Explosive Movements (Jumps, Throws, Sprint) - Main Strength Movement (Squat, Clean, Bench, etc) - Main Assistance Movement (RDL, Lunge, Row, etc) - Accessory Work/Trunk Fast first when fresh Heavy before fatigued Finish with the pump

To any baseball players concerned about their shoulder internal rotation: It is normal to have less IR motion on your throwing side. Your bone is twisted. Your throwing shoulder won't be the same as your other shoulder. It's normal. It's a good thing for throwing baseballs.

The most effective way to improve your rehab skills: Learn how to learn. Set aside time to reflect on your evals, interventions, and plans. What went well? What can you improve? If you never take the time to reflect, you'll never make any progress.
You must meet people where they are and educate with patience. Some of the concerns parents have had recently: Isn’t eating that much protein unhealthy? Isn’t creatine illegal? Won’t lifting weights this young stunt his growth? Aren’t sliders dangerous? It’s ok. Help them.
Tech is great, measuring is great but I think it’s important to remember that players don’t play with tech on their eyes telling them the stats of everything they’re doing & their opponents like they have a Dragon Ball Z scouter on. Athletes don’t experience the game in metrics.

For any sports med clinician who needs to hear it: Rehab is hard. It's not a linear progression. There will be ups and downs, highs and lows. Sometimes the best thing you can do is just bring consistency. Be a constant source of support and encouragement for your athletes.
Programming note: I was ask about following programming on a day to day basis. Meaning- the days I completely follow my exact plan vs days I go off script. I have yet to follow my exact plan I wrote for sessions in years. The only exception would be combine style training.
Programming is incredibly important. But nothing replaces the art of coaching. What we say/not say, do/not do, and decide / remain indifferent in the sessions is the difference for our athletes.
