Memory graph rot is just RAG drift but harder to fix
10 tools, 3 verification systems, 88 eval cases. The entire stack was new to me — Python, LangGraph, LangChain, LangSmith, and PHP (since high school). The hardest part wasn't the code. It was knowing what to ruthlessly cut and when to stop polishing.
Built two things in a week for @gauntletai week 2: a healthcare AI agent on OpenEMR (Python, LangGraph), and a standalone Withings blood pressure plugin for OpenEMR in PHP. Home device data in the EHR — something Epic has but open source didn't — until now.
Firebase, React, Konva, Gemini Flash 2. Most of the stack was new to me on Monday. ~90-95% AI-gen code via multi-agent Claude — planning & coding agents running in parallel. QA half of my brain still won't let me ship until it's solid. The dev half has run out of excuses.

Context evaporates. That's true in chat sessions, in codebases, in test suites. The interesting question is what structures make it recoverable — or prevent the loss in the first place.
Three instances of the same tool, daisy-chained around a bug. The thing that makes this possible is that Claude Code's context lives in readable text files. Not opaque state. Not a binary blob you can't inspect. Legible state is recoverable state. Opaque state is lost state.
Had a Claude Code session lose context mid-problem. UI truncated to half the conversation, agent responding as if the rest never happened. Transcript on disk was still intact — but the session was cooked. So I opened a second session and pointed it at the first one's logs.
Ultimately, the correct failure strategies to depend on where the biggest business costs come from: do they come from incorrectness, for some rigorous and centralized definition of incorrectness, or do they come from downtime (or, perhaps, some combination of the two). 6/n
