When an organization gets so focused on trying to "scale up" their development efforts that they forget to ask if their current development efforts are really something they want happening at scale.
When teams pretend their requirements are so well written that the developers don't need to have a conversation with the customer and users.
I need to check in this working code, but let me just make one more tiny change first. Three hours later ...
That section of code that obviously needs to be changed, but no one is brave enough to discover what might go wrong if they touch it...because there aren't any tests.
When a method name is worded so you can't tell if it does what you want or the exact opposite.
