New nationwide study from Colombia (65M+ healthcare records, 2010–2024) confirms what many suspected: kidney stone disease is not an isolated urological condition — it is a cardiometabolic phenotype. Among 1.8 million stone formers, every comorbidity tested showed significantly higher prevalence vs. non-stone formers: hypertension (PR 2.94), dyslipidemia (PR 2.95), diabetes (PR 2.79), obesity (PR 2.45), ischemic heart disease (PR 2.72), stroke (PR 1.97), and CKD (PR 3.36). The pediatric signal is striking: CKD prevalence ratio reached 18.76 in children aged 0-9 with urolithiasis — driven by monogenic disorders, not metabolic risk, which underscores the need for early genetic workup and lifelong renal surveillance in every child diagnosed with stones. Clinical implication: stone formers deserve systematic cardiometabolic screening — BP, lipids, glucose, renal function — at every encounter. The stone is the symptom.
