@SassyKadiK I would venture to say that the vessel has a history of mechanical problems. A thorough investigation will go back years tracking the maintenance & repair work completed. This will be a complicated & time consuming process. All accidents are preventable if you look close enough.

@SassyKadiK @osbeachwalker I was on a pretty big ship. I saw only two accidents while I was aboard ship for three years. A 9,000-ton ship hitting any fixed object, even a low speed, is pretty devastating. My ship displaced 9,000 tons of water. The ship that hit the bridge was much heavier.
