Psyllium Husk and IBS: What Does the Evidence Say? Psyllium husk is often viewed as a supplement for constipation, but its role in Irritable Bowel Syndrome (IBS) is far more complex. Among fiber supplements, psyllium is one of the most extensively studied and is commonly recommended for IBS because it helps support stool regulation and overall symptom management. Psyllium is a soluble, viscous, gel-forming fiber. When mixed with water, it forms a gel-like matrix in the gastrointestinal tract. This allows it to have a stool-normalizing effect: • In IBS-C (constipation-predominant IBS), it helps retain water in the stool, improving softness and supporting bowel movements. • In IBS-D (diarrhea-predominant IBS), it absorbs excess water in the intestine, helping improve stool consistency and reduce loose stools. Unlike many insoluble fibers such as wheat bran, psyllium regulates water content within the gut and is less rapidly fermented, which may contribute to better tolerability in people with IBS. However, individual responses can vary. Some people, particularly those with visceral hypersensitivity, may initially experience bloating, abdominal fullness, or increased awareness of gas when introducing psyllium. Importantly, this does not necessarily mean bowel function is worsening. Stool consistency may improve while bloating or discomfort persists, which is why response should always be assessed individually. Practical Considerations ✔ Start with a small amount and increase gradually as tolerated. ✔ Ensure adequate fluid intake. ✔ Monitor overall symptom response, not just bowel movements. ✔ Individual tolerance matters. Evidence guides recommendations, but personal response determines whether a strategy is sustainable. Psyllium is not a cure for IBS.
