@DrSpratticus Well I was lucky enough to have access to MRIdian and an RO willing to offer salvage radiation. PSA 2.5 years later is 0.01.
Near weekly occurrence. Pts dropping $20k, and within 1 yr have recurred and have variable permanent side fx. Now have no data how to follow, how to salvage, optimal dose & ADT, biomarkers work? Is scam of the decade, select MDs doing off trial that will be exposed soon enough

Zach, @DrHowardSandler and I spent days at RTOG HQ counting “cores” from path reports. Same thing, even FIR in small sample size PCSM was numerically better with ADT during 1st 10 years of fu.
@AmarUKishan @NRGonc @EORTC @NCCN 2/n The benefit of ADT is irrespective of RT dose from MARCAP (<74 vs >74Gy), EORTC 22991 (70 vs 74 vs 78Gy), and RTOG 0815 (DE-EBRT vs Combo-brahcy) sealed the deal. No interaction by dose and ADT benefit. @alison_tree @rtdess @HimanshuNagarMD
1/n The SOC for men with intermediate risk (IR) PCa getting RT is RT+ST-ADT (4-6 months). @AmarUKishan and I led the 1st MARCAP meta-analysis of >5000 men with >13 yr median fu showing the clear benefit of ST-ADT w/ RT. @NRGonc @EORTC @NCCN

@DrSpratticus We need a well designed randomized trials comparing focal therapy and active surveillance. I'm afraid that the group of patient for whom focal is advised varies widely among those who recommend this therapy.
