Lots of good answers! This is aortic regurgitation from endocarditis. On the M mode you can see fluttering of the mitral valve from aortic regurgitation (Austin flint)
@DrJayMohan @EchofirstB LV systolic heart failure. EPSS (e- point septal separation) in M-mode shows distance of mitral valve and septum > 1 cm, suggestive of LV systolic heart failure. Very likely low EF
