Thrilled to share #PRIMARY2 Trial Results. This is the first randomised trial to show that adding [⁶⁸Ga]Ga-PSMA-11 PET-CT to MRI can safely halve the number of men needing a prostate biopsy. Hot off the press in @TheLancetOncol 🎉 In 660 men across Australian sites with PI-RADS 2-3 but high clinical risk, PSMA-PET: - Avoided biopsy in nearly half (49%) - Was non-inferior for detecting clinically significant cancer (12% vs 16%) - Halved the overdiagnosis of insignificant cancer (32% to 14%) Fewer biopsies, less overdiagnosis, whilst not missing clinically significant prostate cancer: a real step forward for the diagnostic pathway. Enormous congratulations to @ButeauJames @PeterMacCC , who led this as the centrepiece of his PhD and presented it as a plenary at #EAU26 in London. And to my brilliant co-lead @drlouiseemmett - this was a true partnership across our two centres. #ASCO26 CROWN 👑 This is what happens when you find the right target… and then build the right drug. At 7 years of follow-up in CROWN, median PFS with lorlatinib remains not reached. More than half of patients remain progression-free at 7 years (55%), compared with just 3%. 📊 HR for progression or death: 0.19 Think about that for a moment. Seven years after starting therapy, only 7 new PFS events occurred between years 5 and 7 in the lorlatinib arm. As a GI oncologist, I can only look at data like this and wish we had more common, actionable targets with therapies this effective. We are making progress with HER2, FGFR2, IDH1, BRAF, RAS, NTRK, CLDN18.2 and others, but results like CROWN remind us just how transformative precision oncology can be when the biology and the drug align. The future of oncology is not treating all cancers the same. It is finding the right target, in the right patient, at the right time. @OncoAlert @TheGutOncLab @Onco_Nexus

#ASCO26 This one is special. This is the hottest paper of 2026 and potentially in the history of pancreatic cancer. Let’s dive in. RASolute 302: Daraxonrasib vs investigator’s choice chemotherapy in previously treated metastatic pancreatic cancer Abstract LBA5 (soon!) Presentation: May 31, 2026, 3:21-3:33 PM CDT For decades, pancreatic cancer has been where good ideas go to die. We have optimized chemotherapy. We have sequenced chemotherapy. We have celebrated modest gains. But the central driver of PDAC has always been sitting there in plain sight: RAS. More than 90% of pancreatic cancers have oncogenic RAS mutations, and until recently, we had essentially nothing direct to do about it. Daraxonrasib is an oral RAS(ON) multiselective inhibitor targeting the active GTP-bound state of mutant and wild-type RAS. And in RASolute 302, it delivered. Quick hits: 📌 Phase 3 international randomized trial 500 patients with previously treated mPDAC Daraxonrasib vs investigator’s choice chemotherapy 🧬 RAS G12 population 91.8% of patients had RAS G12 mutations 📈 OS in RAS G12 population 13.2 vs 6.6 months HR 0.40 P<0.001 📈 OS in overall population 13.2 vs 6.7 months HR 0.40 P<0.001 📊 PFS in RAS G12 population 7.3 vs 3.5 months HR 0.45 P<0.001 📊 PFS in overall population 7.2 vs 3.6 months HR 0.49 P<0.001 🔥 12-month OS Overall population: 53.2% vs 17.3% ⚠️ Toxicity matters, but this was not just more efficacy for more toxicity Grade ≥3 AEs: 61.8% vs 69.6% TRAEs leading to discontinuation: 1.2% vs 11.2% This is the kind of survival curve we almost never get to see in pancreatic cancer. This validates RAS(ON) inhibition in the most RAS-addicted major cancer. It takes a target we have talked about for decades and turns it into a clinically meaningful survival benefit in a randomized phase 3 trial. The next questions come fast: 1L combinations, maintenance, perioperative disease, sequencing, resistance, toxicity management, and whether this becomes a new backbone. 🚨The first American guidelines for low-dose radiotherapy in osteoarthritis were just published. That’s not a small thing. For decades, European centers have been treating OA with low-dose RT. American patients largely couldn’t access it. No formal guidance. No insurer framework. No roadmap for referring providers. That changes today. The American Radium Society Appropriate Use Criteria, published in the American Journal of Clinical Oncology, define exactly when and how to use LDRT for OA. Radiation oncologists, rheumatologists, physiatrists, and orthopedic surgeons built this together. For insurers, this is the clinical framework they’ve been waiting for. Honored to be a co-author alongside the global experts who made this possible.
